In a stark warning that cuts through the hype of the artificial intelligence revolution, a major new study reveals that AI chatbots frequently provide incorrect and inconsistent medical advice, posing a direct risk to the public. Published on Feb. 9 in the prestigious journal Nature Medicine, the research from the University of Oxford delivers a sobering verdict: Despite their advanced capabilities, large language models are dangerously unready to act as physicians. The findings raise urgent questions about the rapid deployment of these systems in healthcare and underscore a critical disconnect between laboratory performance and real-world human interaction.
The study, led by medical professionals and AI researchers, represents one of the largest real-world tests of AI for public medical guidance. Researchers recruited nearly 1,300 adults from the United Kingdom and presented them with detailed medical scenarios. Participants were split into groups, with some using popular AI models like GPT-4o, Llama 3 and Command R+ for assistance, while a control group used traditional methods.
The results were alarming. Those using AI performed no better—and in some aspects worse—than the control group. Users of AI correctly identified relevant medical conditions less than 34.5% of the time, a rate no better than traditional methods. Strikingly, the control group had 1.76 times better odds of identifying a correct condition than those relying on AI.
A key revelation is the chasm between how AI performs in controlled tests and how it functions with real people. When researchers fed the medical scenarios directly into the AI models in a sterile, automated test, the systems performed admirably, correctly identifying conditions 94.9% of the time. However, this technical prowess collapsed in the messy reality of human interaction.
Lead medical practitioner Dr. Rebecca Payne stated that the findings should serve as a clear warning. AI simply is not prepared to assume the role of a doctor. She emphasized that asking a language model about symptoms can be dangerous, as it may deliver wrong diagnoses or fail to recognize the need for urgent care.
Users often did not know what information to provide the AI, offering partial descriptions. In turn, the AI models proved highly sensitive to how questions were phrased, delivering vastly different advice based on slight variations. In one instance, two users describing similar symptoms of a life-threatening stroke received opposite recommendations from the same AI.
Furthermore, the chatbots typically provided a jumbled mix of accurate and inaccurate information, leaving users to sift through contradictory advice. Senior author Dr. Adam Mahdi called the gap a wake-up call, arguing that AI systems require rigorous, human-involved testing akin to clinical trials before being deemed safe for public healthcare use.
This cautionary research arrives as tech companies aggressively push AI deeper into the health sector and public use accelerates. A November 2025 poll in the U.K. found more than one in three residents have used AI to support their mental health or wellbeing. This trend places immense responsibility on developers and regulators to ensure these tools are safe.
Professional medical bodies firmly maintain that AI should only complement, not supplant, physician judgment, precisely because it cannot perform a physical exam or grasp the full context of a patient’s life. As one report concluded, “While AI has potential to bridge gaps in public health literacy, it also risks spreading decontextualized—and dangerous—information.”
This risk is not theoretical. In a disturbing trend, patients are landing in emergency rooms after blindly following advice from chatbots. These cases reveal how AI’s flawed algorithms can deliver lethal guidance while evading accountability. With AI integration accelerating—from symptom checkers to virtual nursing assistants—misinformation risks loom large. A 2023 study found that language models frequently hallucinate false clinical details, potentially leading to misdiagnoses.
The core issue is one of trust and verification. Large language models are trained on vast datasets scraped from the internet, including sources with known biases, contradictions and inaccuracies. They are synthesizing and repackaging human knowledge at an unprecedented scale.
The study underscores that compressing the complex, experiential art of medicine into a statistical prediction model is fraught with peril. While tech companies emphasize disclaimers, cases reveal how easily those warnings get overlooked. Chatbots frequently misdiagnose conditions and invent false studies. Up to 25% of AI responses are fabricated, yet disclaimers are often omitted, misleading users into trusting harmful advice.
The Oxford study delivers an unambiguous diagnosis: the current generation of AI is sick with inadequacy when tasked with medical care. The promise of artificial intelligence in medicine remains vast, but realizing that promise demands humility and rigor.
“AI medical advice refers to the use of artificial intelligence systems to analyze patient symptoms, provide diagnoses and recommend treatments,” said BrightU.AI‘s Enoch. “This technology aims to reduce healthcare costs and eliminate human bias by automating the clinical assessment process. However, it raises significant concerns about the loss of personal patient-doctor relationships and the potential for AI-driven decisions to impact broader aspects of an individual’s life, such as insurance eligibility and premiums.”
In matters of health, where the stakes are life and death, technological progress must be measured by its genuine capacity to heal, not harm.
